c53ebb66d0 feat: add support for configuring the app shell 
This allows developers to change the shell from /bin/bash to any other when executing
`dokku enter` or pre/post deployment tasks. Ideally we could detect the shells available,
but that is slow and potentially error prone.

7d8c80f0cc feat: Add the ability to disable ANSI prefixes via DOKKU_DISABLE_ANSI_PREFIX_REMOVAL 
To disable the removal, set the value of this variable to `true`.

43592174e6 docs: Remove all references to VHOST files from documentation 
Users should never be in a position where they need to manually edit files managed by Dokku, and should be pushed to using the `domains` plugin where necessary.

cd32180f1a docs: remove potentially bad template examples 
These examples do not include _all_ of the contents of the original template. Rather than having to keep them up to date and confuse users when we don't, lets just remove them. The section above contains a link to the current `nginx.conf.sigil` and should be referenced by our users instead.

d26f8941bb Adds a mention of manually adding nginx entry 
I was working on a Dockerfile deploy that was setup to listen on port 8844.  I wanted to add ssl and couldn't figure out why adding the ssl cert didn't making things 'just work'.  Since the `proxy:ports-add` step I discuss in this commit is run both `under the hood` and `sometimes`, I thought it might be worth issuing a PR for.

b3b1f12e69 Clarify documentation surrounding port management for domain and ssl configuration 
When configuring domains/ssl certificates, the port mappings for an application may be automatically set. This can cause issues for applications which will use non-standard ports - ie. for Dockerfile deploys.
